• Patient first and last name
• Credit Card/HSA card number, expiration, CVV code, Zip Code, and amount paid
• You can print a receipt once the transaction is complete, or you may enter your email for a copy to be sent electronically.
• Please allow 24 hours for the payment to reflect in your account.
