What do I need to bring with me to my first appointment?
Please make sure to bring your physical therapy referral, your ID, and your insurance card. If you have had any imaging (X-Ray, MRI, etc.) please bring the disc or report with you. This will greatly assist our therapists in setting up an appropriate plan of care. Additionally, when making your appoinment, you may supply your e-mail address so we can send your intake paperwork - this makes your first appointment go much quicker! Please plan to be here for about an hour and a half for your initial appointment.
I was in an auto accident. Will you be able to treat me?
We are sorry to hear about your auto accident. We will be happy to treat you. If any insurance other than your health insurance is involved, please let us know as soon as possible. Also, if you have an attorney, we will need their contact information as soon as possible as well.
My insurance does not have a copay, it has a coinsurance. How does this work?
Coinsurance is very common. Since we are not sure exactly how much your portion of the bill will be, we will advise you what your balance is when we know.
When is my copay due?
Your copay is due at the beginning of your appointment. If you need to make special arrangements, please let our Front Desk know before starting your treatment.
What should I wear to my appointments?
Whatever is comfortable for you to move around in. If possible, please make sure the body part being treated is able to be seen.
